Emergency Information Form
An Emergency Information Form must be completed at the start of each school year. This form provides essential contact information in case of an emergency and authorizes the school nurse to administer over-the-counter (OTC) medications such as acetaminophen, ibuprofen, and other medications listed on the form.
Students are not permitted to carry or self-administer OTC medications.
Please notify the school nurse promptly of any changes to your childโs health information, contact details, or emergency contacts throughout the school year.
Emergency Action Plans for School
If your student has a condition such as:
- Life-threatening allergies
- Asthma
- Diabetes
- Seizure disorder
An updated Emergency Action Plan (EAP) must be submitted to the school nurse each year.
If your childโs healthcare provider does not have a standard form, school-provided forms may be used. If a provider submits their own plan, please ensure it:
- Includes clear and specific instructins
- Contains accurate and complete contact information
- Is signed by both the healthcare provider and parent/guardian
For students participating in activities outside the regular school day, please:
- Ensure your child has access to their rescue/emergency medication
- Communicate your childโs needs with the activity advisor or coach
Important
Regularly check expiration dates on medications such as epinephrine auto-injectors (EpiPens), inhalers, insulin, and seizure medications. These medications can lose effectiveness over time and should be replaced before they expire.
